It's quite simple really. If you have changed the chassis/frame for anything than other than a standard replacement of the the original, or you have altered the chassis/frame, or you have changed the type of steering/suspension eg. from reliant to motorcycle then yes it needs MSVA. Even hardtailing a bike requires MSVA if it's welded on. At the moment you will find a lot of "Reliant trikes" some having no connection with Reliant with either short or no MOT on Ebay.

Years ago you could've snuck it through the system if the logbook just said Reliant tricycle & didn't state a body type. As we keep saying though, those days are coming to an end & MOTs will be the first stumbling block. All MOT stations are computer linked directly to DVLA now. If they're presented with a vehicle for testing that clearly isn't what the logbook says it is, (as soon as they enter the registration number in the computer it tells them what it's registered as), they're supposed to refuse the test. When they do that, they have to give a reason. When they tick the box that says, not as stated on the logbook or whatever, you'll automatically get a letter from DVLA telling you to bring your vehicle in for inspection as there's a discrepency in the records. At that point you'll be issued with an order prohibiting you from using the vehicle on the road. That's posted on the Police computer database, so if you pass a patrol car their onboard camera will flag it up as a banned vehicle. You may well also receive a fine. MSVA it properly -it'll save you an awful lot of hassle in the long run.

Goose, I don't remember what type of trike you have, but bear in mind that if the frame has been changed you will also have to have a new vehicle identifier number (VIN)

I think this really depends on what is stated on the V5c' If the trike was correctly registered then no, as the MSVA is not retrospective. Nearly all trikes except 'motorcycle derived' ones using bolt on conversions and those made entirely of new parts and registered as a new vehicle will have a Q registration. Anything other than a Q plate would probably be treated with suspicion.
